Fetlock Joint Lameness: One of the Most Important Causes of Equine Performance Loss

The fetlock joint is arguably the most important athletic joint in the horse. Every stride, whether in a racehorse, showjumper, eventer, dressage horse, polo pony or pleasure horse, places enormous forces through the fetlock.

The metacarpophalangeal (front limb fetlock) and metatarsophalangeal (hind limb fetlock) joints are high-motion joints that undergo stresses equivalent to five to seven times the horse’s body weight during exercise. This remarkable athletic demand explains why the fetlock is one of the most commonly injured joints in equine sports medicine.

As our understanding of equine osteoarthritis (OA) continues to evolve, veterinarians are increasingly focused not only on treating pain but also preserving long-term joint function, athletic longevity and lifetime soundness.

What Is the Fetlock Joint?

The fetlock is often compared to the human ankle, although it functions very differently.

It is a highly mobile shock-absorbing joint that allows the lower limb to hyperextend during weight bearing. This ability contributes significantly to athletic performance but also places enormous strain on:

  • Articular cartilage
  • Synovial membrane
  • Joint capsule
  • Subchondral bone
  • Suspensory apparatus
  • Collateral ligaments
  • Sesamoid bones

Over time, repetitive loading can cause microscopic damage that accumulates faster than the body’s ability to repair it. Eventually, this can lead to synovitis, capsulitis, cartilage degeneration and osteoarthritis.

 

Why Does Fetlock Arthritis Develop?

Many horse owners assume arthritis occurs simply because horses get older.

In reality, fetlock osteoarthritis is often a consequence of repetitive athletic loading and accumulated wear and tear.

Common causes include:

  • Repetitive training and competition
  • Osteochondral fragmentation (“chip fractures”)
  • Subchondral bone disease
  • Condylar stress injury
  • Osteochondral fractures
  • Joint instability
  • Ligament injuries
  • Developmental orthopaedic disease

Importantly, osteoarthritis can develop gradually without an obvious traumatic event. Many horses show progressive deterioration over months or years before obvious clinical signs or radiographic changes appear.

 

The Earliest Signs of Fetlock Joint Disease

One of the most significant advances in equine sports medicine has been recognising that synovitis and capsulitis frequently develop long before severe arthritis is visible on x-rays.

Early warning signs may include:

  • Fetlock joint swelling (effusion)
  • Reduced joint flexibility
  • Stiffness during warm-up
  • Reduced performance
  • Inconsistent or intermittent lameness
  • Pain during flexion tests
  • Shortened stride length
  • Reluctance to collect or extend

Because the fetlock has relatively little soft tissue covering, swelling, joint capsule thickening and soft tissue enlargement are often easier to identify than in deeper joints.

 

Why Early Diagnosis Matters

Historically, veterinarians often relied primarily on radiographs to diagnose joint disease.

Today we know that significant pathology may be present before major radiographic changes become evident.

Modern diagnostics may include:

  • Diagnostic analgesia (nerve and joint blocks)
  • Digital radiography
  • Ultrasound
  • MRI
  • CT
  • Nuclear scintigraphy

These technologies enable veterinarians to identify subtle cartilage injury, synovial inflammation, subchondral bone pathology and soft tissue injury much earlier than was previously possible.

Advanced imaging has demonstrated that many horses with recurrent fetlock pain already have substantial synovial inflammation and cartilage injury despite relatively modest radiographic findings.

 

Understanding the Real Driver of Fetlock Osteoarthritis

For many years osteoarthritis was viewed primarily as a disease of cartilage. Modern research has shown this is only part of the story. Today we recognise that the synovial membrane and joint capsule play a central role in perpetuating inflammation, pain and progressive joint deterioration.

As synovitis and capsulitis become established:

  • Joint fluid quality deteriorates
  • Joint capsule thickens
  • Range of motion decreases
  • Cartilage degeneration accelerates
  • Subchondral bone changes develop
  • Athletic performance declines

This understanding has fundamentally changed how veterinarians approach fetlock arthritis.

 

How Arthramid® Is Changing the Management of Fetlock Arthritis

Arthramid® represents a different philosophy in managing osteoarthritis.

Rather than acting as a traditional anti-inflammatory medication, Arthramid® is a 2.5% injectable polyacrylamide hydrogel (iPAAG) that integrates into the synovial membrane itself.

Once integrated into the synovial lining over approximately one to two weeks, it becomes a permanent supportive tissue framework that augments and supports the joint’s soft tissue structures, including the cartilage.

These findings are important because fetlock osteoarthritis has traditionally required repeated interventions using:

  • Corticosteroids
  • Hyaluronic acid
  • NSAIDs
  • PSGAGs
  • PRP
  • Stem cell therapies
  • Autologous conditioned serum

with variable outcomes and often limited long-term durability.

The longevity of response observed with Arthramid® has helped many veterinarians move towards a more sustainable long-term treatment strategy.

 

Why Performance Horse Owners Are Looking Beyond Corticosteroids

Corticosteroids undoubtedly remain an important treatment option in selected cases.

However, there is increasing interest globally in treatment approaches that support joint health without repeated reliance on corticosteroid administration.

This has been driven by:

  • Greater understanding of osteoarthritis biology
  • Increased emphasis on equine welfare
  • Desire to preserve long-term joint function
  • Regulatory scrutiny in some competitive disciplines

  • Controlled exercise programmes
  • Weight management
  • Rehabilitation
  • Veterinary monitoring
  • Evidence-based intra-articular therapies
